We are contracted providers for Medicare, Medicaid (Public Aid),
and most insurance plans. After your examination, we will submit a claim to your insurance carrier.
You
may be responsible for a "co-payment" at the time of your visit. Your
insurance card will usually indicate the amount of "co-payment" usually between $10 -$60, depending on your plan.
You may also be responsible for any unpaid deductible amounts on your insurance. Most insurance companies do not pay
for the cost of a refraction. A refraction is the testing done to determine your
glasses prescription. Our charge for this service is $50.
If your insurance plan requires, please secure a referral and bring it with
you to your office visit. If you are part of an HMO, POS or EPO you are probably required to have a referral from your
primary doctor in order to see any specialist. If you do not have your referral we will have to reschedule your appointment.
